Orthopedic Institute, PLLC, an Arizona professional limited liability company (the “Clinic”), its affiliated clinics, and Orthopedic Institute MSO, LLC, a Delaware limited liability company (the “Management Company”), its affiliated and managed clinics and surgery centers (Clinic, surgery centers and Management Company will be referred to herein as “Orthopedic Institute” and sometimes referred to herein as “we” or “us”), complies with all applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex.
